Otitis externa in dogs is considered chronic when it lasts a month or more, and it is recurrent if more than one episode occurs within a year. It is a multifactorial condition with a complex pathophysiology and difficult resolution. Medical treatment should consider inflammation and pain relief, as well as rational antimicrobial selection to minimize bacterial resistance. Knowledge of different therapeutic options is essential to reduce the damage caused by perpetuating factors such as bacteria or fungi in dogs with chronic otitis, and to emphasize on treating the primary disease. Currently, new therapeutic alternatives to the use of antimicrobials are increasingly being explored. These include the use of essential oils and natural products such as honey and its derivatives, research on adjuvants such as N-acetylcysteine or EDTA, vaccines, nano-formulations, and the study of new peptides. In this review the different therapeutic alternatives for successful management of chronic and recurrent otitis are analyzed. These alternatives can improve the quality of life and well-being of patients, maximize the treatment effectiveness, and minimize the occurrence of antimicrobial resistance phenomena that affect animal, human and environmental health.
