Approximately 30-40% of patients with focal epilepsy continue to have seizures despite appropriate medical therapy. More recently, increasing numbers of pediatric patients with intractable epilepsy are also being referred for surgical consideration. Recent advances in neuroimaging technology have revolutionized the identification and evaluation of surgical candidates. HS has been seen on MRI in infants as young as 4 months and 2 years of age The most common etiologies of the epilepsies in pediatric surgical candidates are malformation of cortical development and low grade tumor but some patients with childhood onset of temporal lobe epilepsy due to hippocampal sclerosis also present for early surgery. In addition to seizures, developmental issues are also a major concern in children with intractable epilepsy. Early surgery for intractable epilepsy may improve cognitive and psychosocial outcome.We discuss these features and postoperative seizure outcome of a patient with TLE due to HS and malformation of cortical development.
