Abstract

Introduction Randomized studies have shown the superiority of the use of drug eluting stents (DES) over conventional stents (CS). Information on DES implantation in intrastent restenosis (ISR) is scarce in our country. Objective To compare the clinical results obtained in patients that underwent DES implantation versus CS or balloon angioplasty (conventional group) in a high enrolling center in Argentina. Material and Methods All patients that underwent elective angioplasty due to ISR were enrolled from January 2002 through July 2006 and were divided into two groups: DES group (n=116) and conventional group (n=102). Results There were no differences in baseline characteristics, except for a decreased incidence in previous infarction and an increased presence of acute coronary syndrome in the DES group. In the DES group a higher percentage of diffuse lesions was observed, resulting in a higher number of implanted stents (1.1 ± 0.3 versus 0.5 ± 0.4; p < 0.001) and increased total stent length than in the conventional group (35.5 ± 16.5 versus 9.9 ± 7,6 mm; p < 0.001). A similar death, infarction and emergency revascularization rates were observed after 30 days of follow up. During the late follow up, there were less need of new revascularization procedures in the DES group (12.9% versus 24.5%; p = 0.02). No significant differences were observed in the death and infarct rates during the late follow up period. Conclusion The results of the study suggest that the use of DES in a high enrolling center in Argentina is safe and effective compared to other conventional techniques.
