Abstract

Background: Periprosthetic osteolysis leads to component loosening. The goal of this study is to evaluate the periprosthetic osteolysis of cemented polyethylene acetabular component using MRI. Methods: Twenty-two hips (18 patients) with primary total hip arthroplasty (THA) operated from 1999  to 2004 were assessed. In each case conventional radiographs, physical examination and  MRI were obtained.  The cases were divided into two groups: Group A (with loosening): radiograph with radiolucent lines > 2 mm and cup migration (4 cases). Group B (without loosening):absence of radiolucent lines or < 2 mm and no cup migration (18 cases). Results: MRI showed: Group A: periprosthetic osteolysis in 4 cases. Group B: periprosthetic osteolysis in 3 cases,13 cases without osteolysis and 2 cases could not be evaluated owing to the artifact caused by the prostheses. All patients in group A presented clinical signs of loosening; while in group B only 4 cases were found,3 of them with osteolysis in the  MRI. Conclusions: MRI would be indicated to identify periprosthetic acetabular osteolysis in patients presenting pain related to arthroplasty, with negative radiographs.
