Abstract

Until some years ago, ACPAs were considered as the result of an epiphenomenon of the immune response in RA, its detection focused on the diagnosis and classification of patients. It has recently been established that ACPAs play a central role in the pathophysiology of RA. In this review, we present an exhaustive list of these antibodies with environmental genetic factors, their interrelationship with T cell function and their participation in osteoclast differentiation, emphasizing their concentration dynamics, isotypes and modifications just before the development of the AR. We also emphasize other protein modification processes that occur in RA, such as carbamylation. Finally, we highlight the therapeutic implications that seropositivity and seroconversion of these antibodies could have.
