The Jehovah´s Witnesses, for religious reasons, do not accept blood and different blood products. We report a complex intracardiac surgery in a small child of this religion operated without the use of blood or blood products. A baby, 8 months old, weighting 5.4 kg of The Jehovah's Witness religion, with a total anomalous venous return to coronary sinus, with cardiac failure refractary to medical treatment, was operated with extracorporeal circulation without the use of blood, with very good post-operative outcome. Our protocol included: pre- and post-operative treatment with erythropoietin, iron and vitamin complexes, low priming and high performance oxigenator with minimization of the extracorporeal circuits, intraoperative use of aprotinin and reinfusion of all the blood of the oxigenator. The baby was discharged home on post-operative day 6, with an haematocrit of 29%.
