The duration of the anterograde refractory period through anomalous conduction pathways is decisive in determining the ventricular response and the potential risk of ventricular fibrillation in patients with Wolff-Parkinson-White syndrome. An exhaustive exploration of refractoriness by means of programmed auricular stimulation may reveal the presence of supernormal conductivity in AV accessory pathways during a brief early period of the cardiac cycle, allowing for high ventricular frequencies even with a prolonged refractory period of the anomalous pathway or when anterograde conduction seems abolished. This article reviews several examples of supernormal conductivity through anomalous pathways in order to highlight its potential clinical relevance in patients with ventricular preexcitation.
