SAPHO is an acronym that includes synovitis, acne, pustulosis, hyperostosis and osteitis, corresponds to a clinical entity that includes several manifestations that can appear in the same patient simultaneously or successively throughout his life. It has important radiological characteristics when establishing the diagnosis and its origin in genetic multifactorial intervertebral factors, especially in the infectious and immunological genes LPNI2 and NOD2 located on chromosome 18. Among which can be carried out a vascular thrombosis due to hyperostosis and extrinsic compression by the anterior part of the thorax that can be so remarkable as to reach the compression of the subclavian veins and superior cava a soft tissue mass in the mediastinum.
