Resumen

VETTESE, Andrés et al. Tracheal lesions in patients with severe pneumonia due to COVID-19 and prolonged invasive mechanical ventilation. A case series. Argentinian j. respiratory physical therapy [online]. 2023, vol.5, n.1, pp.36-43.  Epub 28-Feb-2023. ISSN 2618-4095.  http://dx.doi.org/10.58172/ajrpt.v5i1.247.

Introduction:

The objective of this study is to describe the clinical course of two adult patients admitted to a hospital of the Autonomous City of Buenos Aires who required invasive mechanical ventilation (IMV) and developed tracheal lesions found by fiberoptic bronchoscopy.

Case presentation:

The first patient, aged 34, was admitted to the intensive care unit (ICU) with severe pneumonia secondary to COVID-19. The patient required IMV and a tracheostomy. He was subsequently diagnosed with a tracheoesophageal fistula. After weaning, he was fitted with a Montgomery prosthesis and voluntarily discharged.

The second patient, aged 55, was admitted to the ICU with severe pneumonia secondary to COVID-19. The patient required IMV and a tracheostomy. Fiberoptic bronchoscopy showed necrosis of the first and second tracheal ring, generating subglottic stenosis and excessive dynamic airway collapse. After weaning, he was fitted with a Montgomery prosthesis and finally transferred to a rehabilitation clinic.

Conclusion:

We described the clinical course of two patients with severe pneumonia due to COVID-19 who developed tracheal lesions.
