Primary non-Hodgkin lymphoma in the central nervous system is rare in children and in AIDS pediatric patients. We report a seven years old boy, HIV-positive, C3 stage, who developed a non- Hodgkin lymphoma in the central nervous system, with leptomeningeal location. The patient started with signs and symptoms related to increased intracranial pressure, his conscience worsened and he became blind. The diagnosis was made through brain biopsy, immunophenotype of B cells in cerebrospinal fluid, and PCR for Epstein Barr virus in the fluid. The boy was treated with intrathecal and systemic chemotherapy. His condition improved during fifteen months, and then he presented a talamic relapse. He was treated with radiotherapy and finally died four months later. In this article we review the literature about this disease, pointing to the exceptional nature of this patient because of his lymphoma exclusively located in the central nervous system, specifically leptomeningeal.
