Facial asymmetry while crying is caused by congenital hypoplasia or agenesis of the depressor anguli oris muscle. Patients with this anomaly usually present droop of the intact corner of their mouths while crying. Though it is easily diagnosed clinically, it must be differenciated from facial palsies of traumatic or congenital origin. This minor anomaly has been associated with several other anomalies, specially with heart defects, in what was referred as Cayler cardiofacial syndrome. Recently, this syndrome was associated with chromosome 22 deletions, being part of the CATCH 22 phenotype. We have reported a case and made a review of this syndrome emphasizing "facial asymmetry" as a valuable sign for suspecting other congenital anomalies.
