Endothelial dysfunction has been defined as an imbalance between relaxing and contracting factors and has been implicated in the pathophysiology of several cardiovascular disorders, hypertension and diabetes. Plasmatic levels of endothelin-1, nitric oxide, von Willebrand factor, fibrinogen, glycated haemoglobin, glycaemia, Insulinaemia, lipid profile and blood pressure in 30 hypertensive and type II diabetic patients and 25 healthy subjects matched in age, sex, body mass index and smoking and dietary habits, were evaluated in order to study the relationship among these variables in the diabetic prothrombotic state. Endothelin-1, nitric oxide, von Willebrand factor and fibrinogen levels were higher in patients than in controls (p < 0.05). There was a significant positive correlation between nitric oxide and insulinaemia (p < 0.05) and between nitric oxide and von Willebrand factor (p < 0.05) in the group of patients. These data suggest that the prothrombotic state in these patients could be determined by the increased levels of endothelin-1, the high but less bioavailable nitric oxide values, the endothelial damage and inflammatory state reflected by high von Willebrand factor and fibrinogen levels associated with dyslipidemia and the insulin-resistance state.
