The average life expectancy is increasing in the world population, and as a result it also increases the number of frail individuals with cardiovascular diseases (CVD), frequently, associated with comorbidities. Fragility is a biological syndrome that reflects a state of diminished physiological reserve and suffering from it leads to a state of vulnerability to stressors, such as cardiovascular surgery. When patients suffering from frailty are exposed to these stressors, they run the risk of a marked, often disproportionate, decompensation, with a high content of adverse effects, procedural complications, prolonged recovery, functional deterioration, disability and mortality. Therefore, frailty has become a high priority issue, because despite its recognition as a fundamental element in the evaluation of older adults with CVD, a roadmap has not yet been established to facilitate its adoption in usual clinic practice. Therefore, we seek to synthesize the existing body of evidence and offer a perspective on how to integrate frailty into clinical practice. The objective of this scientific article is to update the use of fragility scores for preoperative evaluation of patients who will undergo central cardiovascular surgery to know the predictive value of complications, mortality, estimated postoperative time, recovery and need third-party assistance after the completion of the same.
