Background: Evaluate the treatment performed and results achieved in complex post-traumatic soft-tissue lesions. Methods: Twenty-six patients treated between 2004 and 2008, who required complex procedures to achieve adequate soft-tissue coverage were retrospectively evaluated. Patients' age averaged 30 years. The reconstruction procedures performed were: Latissimus Dorsi flap in ten cases, parascapular free flap in seven cases, reverse sural artery neuro-cutaneous flap in four cases, toraco-abdominal pedicle flap in two cases, rotatory soleus flap in three cases, and gracilis free flap in one case. A cement spacer with antibiotic was used in eight cases. Vacuum assisted closure was used in five cases. Results: Follow-up averaged 16 months. Seven patients reported no pain, twelve reported pain when performing certain activities, four reported moderate pain, and one severe pain. Eighteen patients returned to their previous activities. Secondary amputation was performed in three cases. Conclusions: The existence of a wide variety of lesions makes the establishment of treatment protocols difficult. These patients require a multidisciplinary approach. Results are generally satisfactory when an aggressive approach is applied, and defects are covered early.
