Background: Infection at a TKA or THR site is a devastating complication. The "gold standard" in theses cases is the two-stage revision approach. The purpose of the present paper is to show a series of patients implanted with antibiotic-impregnated spacers, featuring unusual characteristics. Methods: 10 cases (4 male and 6 female), with a mean age of 67.7 years (87-45). Using "rapid prototyping" technology, metal-reinforced intramedullary femoral and tibial stems were prepared, also with increased constriction at the tibial post intercondylar notch. Results: The infection was controlled in all cases. No spacer cracks or dislocations were recorded during the study period. The level of acceptance was satisfactory. Currently, relevant aspects regarding spacers are being discussed, many of them controversial: range of motion, cost, roughness, antibiotic release, fracture, dislocation, and need for additional orthotic equipment. Conclusions: Resorting to specific technology it has been possible to utilize large spacers in special cases.
